Responsibilities & Context

Role Overview

The Case Manager is responsible for coordinating and monitoring the overall treatment and recovery process of clients enrolled in our rehabilitation program. You will play a pivotal role in guiding clients through their recovery journey—from admission to successful community reintegration—by conducting assessments, developing individualized treatment plans, and coordinating multidisciplinary care.

Requirement

  • UTC (Universal Treatment Curriculum) training must be completed.

Key Responsibilities

1. Assessment & Treatment Planning

  • Collaborate with the clinical team to develop and update individualized recovery plans.
  • Identify client strengths, risks, and specific recovery goals.

2. Care Coordination

  • Serve as the bridge between psychologists, psychiatrists, medical officers, counselors, and nursing staff.
  • Ensure continuity of care and facilitate referrals to external healthcare, vocational, and social support services.

3. Progress Monitoring & Intervention

  • Monitor client progress toward treatment goals through regular case reviews.
  • Identify recovery barriers and implement proactive, evidence-based interventions.
  • Support clients in building life skills, coping mechanisms, and recovery-oriented behaviors.

4. Family Engagement & Psychoeducation

  • Provide psychoeducation to families regarding addiction, relapse prevention, and treatment expectations.
  • Maintain regular, professional communication with caregivers to encourage active participation in the recovery process.

5. Documentation & Compliance

  • Maintain accurate, timely, and confidential clinical records.
  • Prepare detailed progress reports, discharge summaries, and treatment updates.
  • Ensure full compliance with organizational policies, ethical standards, and regulatory requirements.

6. Relapse Prevention & Aftercare

  • Lead the development of relapse prevention plans.
  • Coordinate effective discharge planning and monitor follow-up recovery support activities.

7. Team Collaboration & Crisis Management

  • Participate actively in case conferences and clinical supervision.
  • Respond appropriately to emotional or behavioral crises, escalating high-risk situations according to organizational protocols.
