Responsibilities & Context

Coordinator (Night Shift-6pm to 3am)

• Schedule and coordinate client care services based on client needs and caregiver availability.
• Conduct assessments and develop care plans in collaboration with healthcare professionals.
• Monitor and evaluate client progress and adjust care plans as needed.
• Ensure effective communication with all relevant departments for cases.
• Other duties will be assigned or as necessary.

COMMUNICATION WITH CLIENT

Computer Operator

Fluent in English Speaking

Good at Microsoft Word and Microsoft Excel
