  • Experience of any medical records related training would be a plus for this position.

Responsibilities & Context

  • To plan and organize departmental work procedure, also main supervision of the Medical Records staff in order to build up quality service and effective team work.
  • To ensure for smooth flow of all clinical information within the hospital and high degree of confidentiality of every single patient record.
  • Ensure the safely storage of all patient record and restriction unauthorized entrance in Medical Records Department.
  • Supervise the scanning process of IPD and OPD records with monitoring the update process in Medical Records file server.
  • Responsible to send regular dengue affected patient information and monthly Death Audit Report Form to DG Health. Also responsible to input OPD, IPD, Emergency Patient data in DG Health website.
  • Statistics of deceased In-Patient list being maintained on monthly basis as well as provided to concerned authority accordingly.
  • Responsible to inform concern OPD and IPD to reduce some common errors, like registration desk (Date of Birth and Name) radiology department, OPD and different nursing station (wrong ID, wrong Name)
  • Police Verification and Requirements: Prepare all kinds of reply as per the requirements of local thana, police, DB, DG health etc.
  • As per the requirements of Nuclear medicine department, scan all documents of particular patient and keep them in the file server.As per the requirements of particular insurance company and Bank the authenticity of death certificate are verified in medical records department.
  • All kinds of patient information is generally updated from medical records (name, date of birth etc)
  • All kinds of request from patient party is being processed from medical records(example-case summary, any kind of certificate, previous documents etc)
  • Get hold of computer skills and knowledge to carryout daily duties and train the staff to improve their productivity.
  • Make orientation of the new member of staff joining in the department by necessary training and briefing.
  • To have compact knowledge of necessary forms using in OPD and IPD within the Hospital.
  • Coordinate with the consultant/specialist for their necessary medical forms and make a format to be given for printing.
  • Monitoring the medical form stock and ordering the necessary quantity according to usage’s frequency.
  • Issue Birth Certificate for the newborn in KTH with the reference of birth information provided by the Consultant Obstetrician.
  • Ensures that no Pile-Up of works occurs in the Medical Records department.
  • Arrange rotation of duties and prepare Duty Roster.
  • Maintain close relationship and cooperation with all departments within the Hospital
  • Ensure that all patient medical file contents are labeled properly and filed correctly, and the tracer system is effective in tracing the movements of medical file released from Medical Records.
  • Prepare the Monthly Mortality Report at the end of the month.
  • To take responsible care at work for the safety of yourself and other person;
  • To cooperate with your employer or any other person in the discharge of any duty, under the act of regulations;
  • To comply with any instruction or measure on occupational Safety and Health as required under the Act or Regulations
  • To carry out any other function/administrative tasks as delegated by the Chief Clinical Development Support or Management of the Hospital.
