Responsibilities & Context

  • Provide residents with mental illness with information about mental health through awareness sessions.
  • Identify and assess and follow-up residents’ mental health needs and mental health status. Follow the organization’s policies to offer the right assistance.
  • Conduct assessments to develop personalized care plans that address residents’ needs in different stages of life journey mentioned as Functionality development phase, Social development phase, Capacity building phase and Livelihood support phase.
  • Organize individual and group psychosocial support sessions for individuals and families with psychosocial disabilities and mental illnesses.
  • Support residents of home in identifying areas of need and goal development, ensuring goals are attainable and time specific, and that the client plays an integral role in the decision-making process.
  • Evaluate client progress and effectiveness of the care plan. Arrange and lead in client case reviews with other service providers. Monitoring participation of individuals in work, skills development activities, home and community.
  • Connecting with client’s family, if available and facilitating interactions if individuals in keen.Promoting individualism and inter-dependence and facilitating self-discovery
  • Maintain regular supervision of PAs and provide specific skill training, grooming along with regular assessments of their progress.Assist with the administrative maintenance of house activities and other related departments.
  • Represent the Program through community outreach and everyday interactions with other community resources, such as community clinics, hospitals, schools, colleges, ward committees, self-help groups, etc. stakeholders
  • Maintain proper documentation and data records related to project activities as defined in the project management and reporting plan.
  • Maintain adequate confidentiality and discipline in client case management and data recording procedures.
  • Frequently participate in suggested workshops / trainings / seminars to improve knowledge and performance level and for completion of set objective of the program according to the organization’s needs.Participate in regular field visits and complete training specific and other field assignments promptly.
  • Be flexible in supporting others and organizational needs in terms of emergency or urgent needs as and when required.
  • Must maintain regular communication with authorized coordinators and senior personnel for information and guidelines regarding client services.Provide support during crises and connect residents to emergency intervention services if needed.
  • Represent the program and collaborate with community resources like clinics, hospitals, schools, and self-help groups.Attend regular supervision sessions, submit weekly or monthly reports, and participate in relevant workshops and trainings.
